Download and print two copies of the recommendation form, fill in your name, sign in the box for one of the waiver options, and give them to two referees to submit for you
All of the above materials can be submitted by mail (to SUREBET, Center for Bioimage Informatics, Carnegie Mellon University, 5000 Forbes Avenue, Pittsburgh PA 15213) or by FAX (to 412-268-9580). The application deadline is March 15th.
